Job Duties: To take responsibility for agreeing and reviewing person-centred action plans for all participantsTo use the Rickter soft outcome measure to set goals & track participant progress To carry out a positive risk assessment on action plan activityTo support participants in identifying education, training and employment opportunities To organise appropriate communication support for participants to facilitate their engagement in individual action plan activity

Qualifications / Experience: RQF level 6 (e.g., Bachelor's Degree) or equivalent level professional qualificationANDMinimum of 1 year's paid experience in the direct provision of education, training, or support ORMinimum RQF 4 or equivalent in a relevant disciplineANDMinimum of 3 year's paid experience as above
