Job Duties: To provide a flexible, localised service to meet the needs of the children/young people with disability, autism or brain injury and their familiesTo identify, assess, plan and facilitate a range of individual strategies and activities to promote personal resilience and improve access to mainstream community provision.To develop their social and communication skills and develop strategies that will assistant them to become more independent.To facilitate home based and community interventions tailored to the individual needs of the children & young people, providing social and emotional support to familiesTo develop a customised programme of community-based activity for the children/young people (and their family if applicable) that will encourage their engagement within local community.

Qualifications / Experience: RQF level 6 (e.g., Bachelor's Degree) or equivalent level in Psychology, Education, Community work, Health & Social Care, or a relevant discipline professional qualificationAND1 years paid experience in working with young people with autism, disability or brain injury to include developing strategies to enhance their social interaction or engagement ORMinimum RQF 4 or equivalent in Psychology, Education, Community work, Health & Social Care or a relevant disciplineAND 3 years' paid experience as above
