Job Duties: To provide a holistic support service to young parents who have previously been in the care of the SEHSCT. You will assess the individual needs of the families and formulate a plan of support, working towards shared goals, whilst providing practical and emotional support to ensure the best outcomes for the whole family.Key responsibilities of this role are:Ensure the safeguarding of the children and advise parents on safeguarding practices.Take appropriate action to escalate issues pertaining to child development or protection.Maintain and appropriately record and document information including maintaining case files and records in relation to all work activity.Support the young people with making appointments, applying for financial support, attending appointments, etc.Assess needs of individuals and develop and implement support plans.

Qualifications / Experience: Minimum one year experience working with young people and/or children.Excellent communication, interpersonal skills, patience, and empathy.Knowledge of child development and protection protocols and confidence to escalate issues.Ability to form appropriate supportive relationships with service users, representatives, and the wider community.Demonstrate resilience and use initiative to resolve issues and follow correct reporting procedures.Demonstrable IT literacy including experience using Microsoft Office applications, email, and the internet.A full UK driving licence and access to your own vehicle is desirable for this role.
