Job Duties: Participate in the provision of a comprehensive clinical pharmacy service including medicines reconciliation, prescription chart review, producing pharmaceutical care plans and attending consultant ward rounds. Act as a clinical role model for pharmacists and demonstrate the ability to provide safe, clinically effective and cost efficient use of medicines to patients.
