Job Duties: As Support Coordinator, you will provide high quality support to children and young people to have a great life.
Qualifications / Experience: NVQ level 3 in Health and Social Care or equivalent* At least 2 years' (or part time equivalent) paid experience of supporting people with learning disabilities and / or autistic spectrum conditions and / or acquired brain injury within a Health and Social Care settingAt least 1 years' experience of development and/or implementation of Person Centered Care / Support Plans.At least 1 years' experience working as part of a team within a Health and Social Care setting.
