Job Duties: The post-holder will work within a multidisciplinary team which assesses, evaluates, and initiates management for patients admitted to Hospital at Home service to improve their outcomes and experience of care.
Qualifications / Experience: Hold full GMC registration and licence to practice.Have completed at least four years' postgraduate training which will be in a relevant training programme, or shall have equivalent experience and competencies with a minimum of six months in Care of the Elderly/ED/GIM or GP.
