Job Duties: Following the assessed need determined by a 'named worker' - plan, develop, implement and coordinate individual packages of day provision and transport within the community designed to enhance and maximise the capabilities and independence of the user.Participate in monitoring and reviewing individual programmes of care with the named worker, and maintain records as required by the Trust.Establish goals on an individual basis so as to ensure maximum social inclusion within one's own community wherever possible.Please visit www.jobs.hscni.net to see Job Description and Personnel Specification.
Qualifications / Experience: 3 years' paid employment experience in the care of adults with a disability.Or:Professional Social Work qualification recognised by NISCC.Or:First Level Nursing qualification on Part 1 level 1 of NMC register (learning disability, mental health or adult).Or:Allied health professional registered with the Health Professional Council.Or:NVQ Level II in Care, plus 2 years' paid employment experience in the adults with a disability.Please visit www.jobs.hscni.net to see Job Description and Personnel Specification.
