Job Duties: Creation/development/monitoring of live PCPs/roadmaps for all clients of NHP. Ensure info held on each service user is accurate and cognisant of GDPR. (Creation/maintenance of a bespoke database linking into the trainees Goal Tracker/PCP evidence. Link person between each client, the family with NHP Ltd. Reporting of the progress of each trainee: checking/collation of all monthly reports/submission to funder. Checking completion of PCP Tracker tool monthly. Contact teaching time with LD students in service. Monitoring/reporting trainees attendances /absences/new starts/leavers etc. All Company communication-letter, text etc. Prep for, coordination of and recording of annual reviews-sharing information to ensure that all are fully informed/actions agreed. Ensure H&S of clients at all times-creation/updating of live risk assessments for each client in each environment in which he/she receives support Report A&I C&C- complete all required written records as required.
Qualifications / Experience: Third level qualification in education/health and social care/other relevant discipline with min 2 years paid employment, within the last five years, working in the field of learning disabilities Experience of facilitating and implementing a person-centred plan/experience in understanding LD and familiesExcellent time management, organisation and communication skills/ adaptability/ flexibility /willingness to approach new challengesAbility to develop and maintain effective professional relationships with all of the staff TeamHold a high level of proficiency in using the full Microsoft Office Suite, particularly Word, Excel. Access and PowerpointAccess to a car which is insured for business use and hold a current driving licence. (Consideration will be given to alternative travelling proposals in respect of applicants with a disability who cannot hold a licence)
