Job Duties: Work as part of team to assist in the provision of social care within the community/family home setting.Contribute to assessment, planning, implementation, evaluation and reviewing duties in relation to the development of care plans for young peopleUndertake the direct care of children and young people, which involves ensuring that their social, emotional, health, educational, spiritual and physical needs are met as fully as possible. Direct care may include practical tasks such as support with personal care, cooking and supporting eating and drinking and serving food on occasions.
