Job Duties: Act as the 'designated officer' for all applications for assessment and Care ManagementSet in place an effective system for screening referrals to ensure that people who meet the eligibility criteria for a comprehensive assessment are able to access the help they need.
Qualifications / Experience: Be a registered Nurse, Part 1, (Mental Health, Learning Disability) on the current NMC register.ORHold a Social Work Qualification on the current NISCC register, (CQSW, Dip in SW, CSS)ORHold an Allied Health Professional (AHP) Qualification on the current HPC register.ANDHave a minimum of 3 years in a Health and Social Care setting with mental health service users.
