Job Duties: To use advanced clinical reasoning, demonstrating critical thinking, reflection and analysis to develop appropriate behavioural and falls management strategies for residents experiencing acute behavioural disturbances or /and falls associated with their condition to prevent hospital admission.To undertake care home visits to review self-maintenance and leisure skills and how cognitive/ frailty problems may be affecting daily functioning and to provide strategies for the care staff that promote the physical health, wellbeing, mobility and independence of residents.Please visit www.jobs.hscni.net To see Job Description and Personnel Specification.
