Job Duties: Case Manager Nurse Community Learning Disability Team. Undertake clinical/therapeutic activities, nursing procedures and prescribed treatments in a safe and professional manner.Undertake nursing assessments and liaise with multi-disciplinary team colleagues to commission specialised assessments.
Qualifications / Experience: Registered Learning Disability Nurse (RNLD Level One) on the Live NMC Register.Hold a current full driving licence which is valid for use in the UK and have access to a car on appointment.
