Job Duties: To work in partnership with identified Primary Health Care teams, GP Practices and learning disability teams.To work as an integral part of the Community Nursing Learning Disability Team.

Qualifications / Experience: Applicants must be a Level 1 Registered Nurse (Learning Disability Branch) on NMC Live Register.3 years post qualifying experience working in a nursing role within learning disability care settings.
