Job Duties: Lead in providing a person-centred holistic approach to assess both the physical, psychological and social needs of the patient/client, families and carers.Act as a specialist expert resource in the planning and communicating of complex/highly complex programmes of care to all disciplines and in partnership to patients/clients, families and their carers, to support timely access to relevant care.
Qualifications / Experience: Registered nurse: first level or BSc(Hons) in Nursing and live on NMC registerA Minimum of 5 years' post registration experience as Band 5 or above within an acute or community setting caring for patients with diabetes
