Job Duties: Practise autonomously, using a person and family centred approach, within the expanded scope of practice and the wider health care team. Undertake comprehensive assessments including HEF.Refer for investigations, make diagnoses, prescribe care and treatment for example those with acute illnesses, exacerbation of long-term conditions or review of palliative and end of life care needs.
