Job Duties: Work in close liaison with Acute and Community Rehabilitation Services to facilitate day to day co-ordination and screening at the Acute / Community Interface. Co-ordinate regional discharges to community bed based services or to the most appropriate pathway. Daily attendance at Acute Hospital Sites to review EOA episodes. Qualifications / Experience: Registered Nurse on Part 1 of the Live NMC register with 2 years' experience at Band 6 or above in either a hospital rehabilitation or community setting.Or:Professional social work qualification recognised by NISCC with 2 years' experience at band 6 either in a hospital rehabilitation of community setting (By date of appointment must be listed on the NISCC Register of Social Workers).Or:Allied Health qualification with 2 years' experience at band 6 or above within a hospital rehabilitation or community setting.Please visit www.jobs.hscni.net to see Job Description and Personnel Specification.
