Job Duties: To accept direct referrals from multi-disciplinary team members within the acute hospital setting and Community stroke team in accordance with the referral criteria set out for the service.To independently manage a caseload of patients with complex and highly complex needs evidencing a high level problem solving and reasoning skills approach.
