Job Duties: Act as a key worker for patients with Skin cancer offering an accessible point of contact and coordination of care across the patient journey, consistent with the responsibilities outlined in the Improving Outcome Guidelines. Within a nurse provided service, undertake advanced holistic assessment at key points in the patient pathway, and as required. This could include physical, emotional, psychological, social and spiritual needs and the development of appropriate plans of care and appropriate onward referral. Provide continuous ongoing assessment of information and support needs of patients and carers, signposting to other sources of information and support, referring to specialist services when required.

Qualifications / Experience: Registered Nurse on Part 1 of the NMC Register (General Adult Branch).Be in possession of a degree in Nursing or other health related subject. Be in possession of a qualification in a relevant clinical specialism.
