Job Duties: The postholder will;Facilitate, support and advise primary care professionals on implementing the new FPS contracts and in developing high quality services for patients.Work with primary, community and specialist services to improve integration of sectors and services in a locality. Qualifications / Experience: University degree or equivalent professional qualificationANDAt least two years experience, which should include recent experience in the Primary Care field.ORAt least five years experience, which should include recent experience in the primary Care fieldAND2A.
