Job Duties: Enable people with learning disabilities to live fulfilling and valued lives and work within the Community's ethos and values. Provide individualised care and support in accordance with the needs and preferences of the individual in a safe and compassionate way. Ensure effectiveness of care and support provision by working to individual support, care and management plans.
Qualifications / Experience: NVQ/QCF Level II in the Health and Social Care or allied professional qualification.
